- The number of mesothelioma deaths has increased from 153 in 1968 to 2 321 in 2009. Over 80% of deaths were among men. (Mesothelioma register).
- Men who worked in the building industry when asbestos was still being used widely are now among those most at risk of mesothelioma.
- Most deaths among women are likely to be due to a range of non-occupational asbestos exposures.
- The worst case predictions for males are that annual deaths will increase to a peak of about 2 100 in about the year 2016.
- Female deaths are likely to peak after this but at a much lower level.

Mechanisms which could be activating meso.
Layard and Stanton hypothesized in 1977/78 that chemical effects linked to cancer (mesothelioma) are not initiated by the toxicity of fibrous materials, that's any trigger-effects of asbestos must presumably be physical. They presume:
(A) Mechanical Damage. There is experimental evidence that very slim fibers (<60 nm, <0.06 μm in width) do tangle destructively with chromosomes (being comparably sized ). It is quite clear that it's likely to cause the sort of mitosis disruption expected in cancer (mesothelioma).
(B) Unwanted Signal Channels. This presume has recently been explored theoretically, but unfortunately not yet experimentally. The theory claims that this effect would only be feasible for asbestos fibers >100 nm in width ( or >150 nm in the chrysotile case), suggesting that we should be on the look-out for a possible mixture of different mechanisms for the various fiber-diameter-ranges.

Although this may be true, it does't explain → → (2) the actual trigger.

- After all, inflammation is commonly seen as caused by chemical-based processes: immunological and/or bacterial. So inflammation may well be part of the causal chain, but not a crucial first step.

What is asbestos?
Asbestos is a set of six naturally appearing silicate minerals used commercially for their desirable physical properties

The principal forms of asbestos are white asbestos (chrysotile) and blue asbestos (crocidolite).
Crocidolite and amosite are the most dangerous of the asbestos minerals set because of their long persistence in the lungs of exposed people. Chrysotile is often contaminated with tremolite asbestos, thus creating an additional hazard. Chrysotile asbestos, can produced tumors in animals, like all other forms of asbestos minerals.
Asbestos crystals structure, make him different from most other crystalline minerals. Asbestos crystals become flexible, long, silky fibres, so it can be made into a wide variety of forms. Because it is resistant of high temperatures, his extraordinary tensile strength, and relative resistance to chemical attack, asbestos is used commercially for insulation in buildings and as an ingredient in a number of products, such as fire blankets, medical packing, plastic fillers, roofing shingles, and water supply lines, as well as clutches and brake linings, gaskets and pads in automotive industry.
In nature asbestos can be found in the air outdoors, indoors and in some drinkable water, including water from natural sources. Actual indoor and outdoor concentrations in the air range from below one hundred to several thousand fibres per m3.

Raw asbestos crystals will split into individual fibers which are large enough to be seen, but they can split even further into microscopic fibers. These small fibers are flying in the air, and can even pass through some respiratory dust filters.

When asbestos fibres are inhaled , they may cause scars in the lung tissue, cancer of the bronchial tree (lung cancer) and sometimes cancer in the pleura and peritoneum. If asbestos fibers are inhaled in a high concentration over a long time period of time it is more likely to cause some serious health problems. This is most common among asbestos miners, because these workers have the longest exposure to it.
To humans, all forms of asbestos are carcinogenic, and may cause mesothelioma, lung cancer, larynx and ovary cancer. Asbestos exposure is also responsible for other diseases, such as pleural plaques, asbestosis, thickening and effusions.
Patients who become ill from inhaling asbestos fibers are often those who are exposed on a day-to-day basis in a job where they worked directly with the asbestos materials. As a person's exposure to fibers increases, then that person's risk of the disease also increases, because of being exposed to higher concentrations of fibers and/or by being exposed for a longer time. Mesothelioma cancer and other asbestos related diseases are very unlikely to result from a short period of exposure to lower levels or from a single, high-level exposure.
Mesotheliomas have been observed on people who were occupationally exposed to white asbestos (chrysotile), population who lived close to asbestos mines and factories, and family members of the occupationally exposed. According to the NCI, "A history of asbestos exposure at work is reported in about 70 percent to 80 percent of all cases. Asbestosis has been reported primarily in asbestos workers, and appears to require exposure to a high concentration over a long time period for the development of the clinical disease. There is also a long latency period of over 12 years. Some studies have shown an increased risk of lung cancer among smokers who are exposed to asbestos compared to nonsmokers.
Health Canada states that the asbestos content of a product does not indicate its health risk. Asbestos is hazardous to health only when asbestos fibres are in the air that people breathe.
The evidence that ingested asbestos causes gastrointestinal or other cancers is insufficiently proven. The carcinogenic properties of asbestos are most probably due to its fibre geometry and remarkable integrity; other fibres with the same characteristics may also be carcinogenic.
Current environmental concentrations of asbestos are not considered a hazard with respect to asbestosis. A risk of mesothelioma and lung cancer from the current concentrations can't be excluded.
Studies also shown that members of the general (non-occupationally exposed) population have tens of thousands to hundreds of thousands of asbestos fibers in each gram of dry lung tissue, which translates in millions of fibers and tens of thousands of asbestos bodies in every person's lungs. Health risks associated with exposure to NOA (naturally occurring asbestos) are not yet fully understood, and current US federal regulations do not address exposure from NOA.
Has the use of asbestos containing products changed during the years?
After it became evident that regular exposure to asbestos products on the job involved health risks, the public became more concerned about exposure to asbestos in offices and schools, and, eventually, about all asbestos products. This concern has led to a dramatic decline in asbestos products use since the early 1980s. The use of asbestos insulation products in heating systems and buildings has practically disappeared. Using asbestos products for roofing, appliances and flooring, also continues to decrease.
Today Hazardous Products Act regulates the sale of asbestos containing products. Asbestos is now better closed and sealed to reduce the escape of fibres. Asbestos is still valuable in many applications because it has been difficult to find comparable substitute materials. For example, it is an important component of clutch facings and brake lining.

Introduction
Malignant pleural mesothelioma (MPM) is an aggressive tumor, usually associated with a poor prognosis. The incidence of MPM is increasing throughout most of the world, and it is expected to rise in the next 10–20 years as a result of widespread exposure to asbestos in the past decades. The management of patients with MPM is controversial. Difficulties in diagnosing and staging, especially in early disease, have thwarted the development of a generally accepted stage-related approach. Staging
Correct clinical staging is mandatory in the approach to treat MPM. Early staging systems have reflected mainly the experiences of individual institutions on their respective patient populations; considerable discrepancies among the various systems have resulted in nonuniformity of reporting. Chemotherapy
Most patients with MPM are potential candidates for chemotherapy at some point in their treatment. For many years a nihilistic attitude has existed about medical treatment of MPM. Most chemotherapy studies have used either single agents or combination regimens in the setting of small phase II trials; the rates of objective tumor regression have been generally <20%, with no significant impact on median survival. In a meta-analysis of early trials published between 1965 and 2001, cisplatin was found to be the most active single drug. Types of Mesothelioma
Mesothelioma Basic Terms
Let's first go back to basic terms associated with mesothelioma. Basically, every cancer, starts from the tumor mass in our bodies.A tumor is caused by body tissues that grow to form an abnormal mass. There are two basic types of tumors.
- Benign type is a non-cancerous type of tumor.
- Malignant is a cancerous type of tumor.
The terms tumor and cancer are sometimes used synonymously which can be misleading. A tumor does not necessarily mean a cancer. The term tumor simply refers to a mass. It is helpful to keep these distinctions clear when we are discussing a possible cancer diagnosis. The term cancer refers to a new growth which will invade surrounding tissues, metastasize (spread to other organs) and may lead to the patient's death if untreated. The word cancer in Latine means crab. Like a cancer, a crab grabs onto something and will not let go.
Benign Mesothelioma is a tumor usually localized in the mesothelial layer around the lungs, heart, stomach or elsewhere in the abdomen and grows larger. However, it is not cancerous and rarely spreads to nearby tissues or other body areas. Benign mesothelioma symptoms are often similar to those of other types of mesothelioma. Treatment for benign mesothelioma tumor is usually surgery. The tumor responds very well to treatment. However, if left untreated, it can lead to a serious disease.
Malignant mesothelioma is a general term that means the mesothelioma is a cancerous type. All forms of cancer are malignant.
Based on cancerous cell types, mesothelioma is mainly classified into epithelioid, sacromatoid and mixed/biphasic.
- Epithelioid mesothelioma type is the most common form of mesothelioma cancer cells and accounts for about 50-70 percent of all people diagnosed with mesothelioma. It has the most positive prognosis (chance of recovery) among all other types, because it may grow slower and is more likely to respond to treatment. This form of mesothelioma targets the thin membrane surrounding the body's internal organs and surfaces.
- Sarcomatoid mesothelioma type is more aggressive and affects the body's secondary tissues - muscles, bones, fats and cartilage. It is found in about 7-20 percent of all mesothelioma cases and has a worse prognosis. It generally does not respond well to therapy.
- Mixed / biphasic mesothelioma type accounts for approximately 25-30 percent of all diagnosed mesothelioma cases and contains both sarcomatoid and epithelioid types. This mesothelioma type has an intermediate prognosis.
Mesothelioma Classification By Body Parts
Another classification of mesothelioma cancer is based on the areas /parts of the body affected by the disease . There are four main types of mesothelioma.Pleural mesothelioma
This type of mesothelioma cancer is the most common since it affects the pleural region of the chest cavity. Pleural Mesothelioma cancer makes up nearly 75 percent of all Mesothelioma diagnosed cases. This disease usually strikes the pleural cavity located between the thin membranes surrounding the lungs, causes pressure on it and trouble breathing as fluid builds in the chest cavity and the pleural lining thickens. That diminishes the lungs' air capacity and normal contraction and expansion functions.
Peritoneal mesothelioma
Another, second most common type of mesothelioma cancer is the peritoneal mesothelioma cancer, accounts around 20 percent of all mesothelioma cases. This disease affects the peritoneum lining of the abdominal cavity, gastrointestinal tract, bowel, liver and spleen, causing a build up of fluid in the abdominal cavity as cancerous cells spread. The lining surrounding vital organs in the abdominal area cannot function normaly and patients who suffers from peritoneal Mesothelioma cancer can experience symptoms of pain or swelling in the abdomen, anemia, breathing difficulty, blood clotting, or restriction of properly bodily functions like digestion and bowel movements.
Pericardial mesothelioma
Pericardial mesothelioma is the rarest type of the three main mesothelioma cancers and affects the pericardium, the protective sac that covers the heart and the major arteries and veins. The pericardium normaly provides constant lubrication so that the heart can function properly. Pericardial mesothelioma is more rare than pleural or peritoneal mesothelioma forms, but it is in the same time more dangerous. Often this type of mesothelioma cancer is discovered too late for treatment, and even when detected early only few treatment methods exist. Symptoms include coughing, heart palpitations, shortness of breath, irregular heartbeat and chest pains .
Testicular mesothelioma
Testicular mesothelioma cancer, has been reported in less than 100 cases. Unfortunately, this means that there are no common symptoms reported across these cases. Discovering lumps on the testicles and the swelling of the scrotum often lead to a testicular mesothelioma diagnosis.
Early detection of Mesothelioma is very important
All types of mesothelioma are hard to detect during their early stages of the disease, so the earlier that mesothelioma is detected, the more treatment options are available. Early detection can provide quicker treatment and disease management, as well as the greatest chance at a better quality of life. All types of mesothelioma cancers are very difficult to diagnose, in a large part because they mimic the normal symptoms of other more common diseases. Surgery is typically performed to remove the cancerous mass. Patients may also undergo radiation and chemotherapy to reduce or eliminate the remaining cancer mass, although in many cases the mesothelioma is deeply entrenched in the tissue and cannot be completely removed.
There are some other forms of mesothelioma, which are very rare, such as Papillary mesothelioma. Deciduoid mesothelioma, cystic mesothelioma, etc.
